Take Interstate-12 to exit 40 (Ponchatoula / Hammond).  Our Hammond urology office is  1 mile south of Interstate-12 on Veteran's Avenue.  The Hammond office is in the Veteran's Plaza building directly across the street from North Oaks Medical Center.
