NAD+ (nicotinamide adenine dinucleotide) is a naturally occurring enzymatic cofactor which is universally present in all cells of the human body. It serves many essential biological functions, primarily including the breakdown of food stuffs into cellular energy resulting in the production of ATP (adenosine triphosphate), the biological compound which is the basic unit of energy currency for healthy cellular function.
Not only is NAD+ critical for body energy stores, it also interacts with other important metabolic protein systems that influence health and longevity. Sirtuins are key NAD+ dependent enzymes responsible for a multitude of health benefits by impacting metabolism, cell cycle regulation, and possibly even longevity. Pterostilbene, an antioxidant compound found in blueberries, is a potent sirtuin activator which as a dietary supplement has been studied in combination with NR. Through interactions with sirtuin proteins, both NAD+ and pterostilbene appear to influence inflammatory response, cellular proliferation, antioxidant defense, protein production, insulin secretion, and cancer prevention. Likewise, through interactions with poly adenosine diphosphate-ribose polymerase (PARP) enzymes, NAD+ influences other critical biological processes including DNA repair, chromosomal stability, and apoptosis (programmed cell death).
NAD+ levels decline naturally with age, just as many disease processes increase with age. While tissue NAD+ levels decline with age, the incidence of fatigue, inflammation, vascular disease, metabolic disease, infertility, and malignancy increases with age. One scientific review states, “… as NAD levels taper off, the mitochondria are unable to produce the (cellular) energy (as) they once did; this in turn leads to improper cell function that then leads to biological decay such as cardiovascular disease, cancer, neurological disease, lung disease, (and other diseases).”
Women are born with their lifetime maximum compliment of eggs (oocytes) within the ovaries, producing no new eggs thereafter. As such, human eggs are thus exposed to accumulating biological stressors with each passing year of life, as the number and quality of eggs slowly diminishes with age. Egg quality decreases, as NAD+ levels fall.
Diminished ovarian reserve (DOR) is a medical term describing declining egg numbers and quality. While the condition may occur at any age, DOR is more commonly diagnosed as women postpone childbearing beyond age 30. DOR is linked to poor fertilization and conception rates, and in animal studies, DOR is linked with diminished oocyte (egg) NAD+ levels.
It is known that supplementation with NAD+ precursors (molecular building blocks), such as nicotinamide riboside (NR), results in higher intracellular NAD+ levels. Supplementation trials describe NR as “well-tolerated, up to 2 grams (2000 mg) daily dose” and not associated with side effects like flushing (as can be experienced with the use of other NAD+ precursors).
Higher NAD+ levels (as potentially achieved via NR supplementation) may preserve or improve egg quality, thereby improving fertility and pregnancy rates. Alternative methods of increasing cellular NAD+ remain under investigation but may ultimately prove to be complementary to NR supplementation. Yet, the current state of scientific knowledge is incomplete with regard to the complex biochemical nature of NAD+ metabolism, NAD+ precursor supplementation, and potential utilization of complementary pharmacological manipulations of NAD+ biosynthesis.
A multitude of possibilities exist for progress within this rapidly expanding field of regenerative medicine. Hope abounds that age-related health conditions including the challenges of diminished ovarian reserve and multiple inflammatory age-related medical problems may be stabilized, improved, or even reversed. At present, supplementation with the widely studied NAD+ precursor, nicotinamide riboside (NR), together with the well-known antioxidant, pterostilbene, may represent the most practical way of increasing NAD+ levels and confronting age-health-related diminished ovarian reserve.
Could raising NAD+ levels with NR supplementation counterbalance age-related NAD+ decline, extend lifespan, and repress the ill-effects of DOR and other age-related diseases? Is NAD+ truly the molecule of youthfulness? Scientific answers are incomplete, but ongoing research in the rapidly expanding field of regenerative health and life extension should soon provide some long-awaited answers.
